Your symptoms sound like heart failure. You may need a heart ultrasound. At the same time, you need to take diuretics under the guidance of your cardiologist.
Shortness of breath and ankle swelling are typical symptoms of heart problem, especially heart failure. In this case, the heart just cannot work well and this causes fluid accumulation in your body, such as the lung (that's why you have shortness of breath) and the lower legs/ankles.

I think maybe you need to see your primary care physician or a cardiologist (heart doctor) to get tests. You can restrict daily water intake and elevate your legs while sleeping to lessen the symptoms.
