Abdominal pain with vomiting is a common sign. I don't think it could cause pain in the back.
First signs may include:

Nausea and vomiting
Abdominal pain
Weakness or fatigue
Shortness of breath
Fruity-scented breath

Back pain is not a typical symptom.

It might be ulcer. However, other conditions with similar clinical symptoms should also be considered, such as gastritis, gastroesophageal reflux disease, pancreatic cancer, gallstones.

I don't think so. Diabetic ketoacidosis generally does not cause back pain.
Yes it does!  My granddaughter had ketoacidosis and as I drove her to the hospital she screamed with pain in her back (kidneys)!  The doctors said if she had gotten there a few minutes later, she would have died!  She was in ICU for a week, but she survived!  Ketoacidosis is a dangerous condition!
Hi, Linda, I have seen a lot of people with abdominal pain and they were often proved to be diabetic ketoacidosis patients. I have never seen one diabetic ketoacidosis patient with a back pain. The exact reason for this pain is poorly understood. I do agree the ketoacidosis is a life-threatening condition that should be diagnosed early and  treated promptly.

My 4 year old son was diagnosed yesterday. In the past few weeks, he started drinking a ton of water! Over the next few days, he started feeling sick. Then he started vomiting, running a fever. The next morning, he didn't even know who he was! I was at a loss! You know, I am a single mother. Then with the help of my neighbor, he was taken to the closest children's hospital. Awful!
