Hiatal Hernia is an annoying disease. The patient can first try medications such as prokinetics to promote gastrointestinal motility and antacid such as PPI or H2 receptor antagonist. If the medications do not work, you are suggested to see a surgeon for surgery treatment.

Have you taken some medications? If you are affected by heartburn and acid reflux, you can try to take drugs such as antacids that neutralize stomach acid (Mylanta, Rolaids and Tums) or medications that block acid production and heal the esophagus (proton pump inhibitors including lansoprazole and omeprazole). Surgery is generally used for people who aren't helped by medications to relieve heartburn and acid reflux, or have complications such as severe inflammation or narrowing of the esophagus.

After many years of terrible pain I finally saw a gastro enterologist about my epigastric pain.  You should have an EGD  (scope of your esophagus and stomach) to make sure that you do not have a condition called Barrett's esophagus.  This occurs after years of the acid in your stomach washing into the esophagus.  This washing is caused because the hiatal hernia does not allow your lower esophageal sphincter to function properly.  At the base of your esophagus there is a muscle that closes so the acid stays in the stomach.  Take your index and middle finger and grab them with your other index finger and thumb.  The fingers on the first hand represent the esophagus.  The fingers on the other hand represent where the esophagus goes through the diaphragm.  In people with out hiatal hernia there is gentle pressure around the esophagus.  To see what it looks like without that pressure open your second hand enough to see a little bit of an open ring around the first fingers.  This open ring is a hiatal hernia.  Without the gentle pressure, acid washes up into the esophagus and irritates the lining of the esophagus.  This is what causes the pain.  Medications are useful to a lot of people, but some cases do not respond to medications.  If this is your situation please see the Gastro-enterologist.  I ended up having surgery to fix the problem and it is the best thing I ever did.  You might want to ask the GI specialist about this if other measures do not give you relief.
